2. Fields : Graduate School (Master and Ph.D.), Graduate School of Industrial Management (Master only),
                 Graduate School of Health Science & Welfare (Master only), Contract-based Department

3. Required Documents : Refer to the attached documents.

5. Notes :
   a. Late arrival of the application documents after the deadline (17:00 November 15th) will result in failure of your
      admission application.

      application.
   b. If you are submitting the application via mail, make sure to enclose the application fee(postal order) in your materials
      and necessarily send it by registered mail.
